HOW TO REQUEST ONE
I. Fill out the request form below.
II. Our team will review your request.
III. If approved, we’ll prepare and ship your care package.
IV. You’ll receive a confirmation and tracking information.

Care packages are provided at no cost to the recipient.
While supplies last. One package per patient. Allow 1-2 weeks for delivery.
